SMITH, Phil A. Husband of Patty Smith (nee Boyd). Father of Stephanie Gayle, Christopher Paul and Sarah Elaine Smith. Survived by his loving dog Shelly. Son of Paul (Sharon) Smith and Janet Kuhn. Brother of Richard (Michele) Smith, Carrie (Tim) Cain and the late Lisa Burleson. Also survived by several nieces, nephews, great nieces and great nephews. Passed away Friday, October 19, 2012, at the age of 50. Visitation at PromiseLand Church, 6227 Price Rd., Loveland, OH 45140, Thursday 5-9 p.m. Funeral service Friday 11 a.m. at the church.
